可透過網站的預約申請頁整理服務、希望日期及時段，再以晴朗商場診所 WhatsApp 9232 7056 送出查詢；亦可於營業時間致電 2336 8018。煥然壹居診所的開診及預約安排，請致電 9883 2163 查詢。晴朗商場診所位於啟德承啟道28號德朗邨晴朗商場B區地下B15號舖，百佳樓下；星期一至五08:30–21:00、星期六及日08:30–18:00，公眾假期休息。個別醫生當值時間可能不同，到訪前請核對醫生及診所。查詢不會自動保留時段，輪候時間亦視乎當日情況。

Prepare your service, preferred date and time on the appointment-request page, then send an enquiry through Ching Long Clinic’s WhatsApp on 9232 7056, or call 2336 8018 during opening hours. For De Novo Clinic opening and appointment arrangements, call 9883 2163. Ching Long is at Shop B15, G/F, Ching Long Shopping Centre Zone B, Tak Long Estate, 28 Shing Kai Road, Kai Tak, below PARKnSHOP. Hours are Monday–Friday 08:30–21:00 and Saturday–Sunday 08:30–18:00, closed on public holidays. Clinician sessions can differ; confirm the clinician and location before travelling. An enquiry does not hold a slot, and waiting depends on the day’s circumstances.
求診時需要帶備甚麼？What should I bring to the consultation?
求診時建議帶備：
香港身份證或其他身份證明文件
適用的醫療卡、保險卡或長者醫療券資料
正在服用的藥物或完整藥物清單
已知藥物、食物或其他過敏資料
相關的化驗報告、影像報告、出院摘要或轉介信
疫苗接種紀錄或兒童健康紀錄
家中量度的血壓、血糖、體溫或其他健康紀錄
症狀開始的時間、變化及曾使用的治療資料
兒童、長者或需要協助的人士，可由熟悉其健康狀況的人陪同。
除非診所已特別通知，求診前一般不需要自行禁食。如預約了指定健康檢查或化驗，請按照診所提供的準備指示。
Please bring the following when available:
A Hong Kong Identity Card or another identification document
Any applicable medical card, insurance card or Elderly Health Care Voucher information
Current medications or a complete medication list
Details of known medication, food or other allergies
Relevant laboratory results, imaging reports, hospital discharge summaries or referral letters
Vaccination records or a child health record
Home blood-pressure, blood-glucose, temperature or other health records
Information about when the symptoms began, how they have changed and any treatments already used
Children, older people or patients who require assistance may be accompanied by someone familiar with their health history.
Do not fast before a consultation unless the clinic has specifically instructed you to do so. Patients attending a particular health screening or laboratory test should follow the preparation instructions provided by the clinic.
